Scholarships Available
Middle Park Medical Foundation offers scholarships to Middle Park Health Employees, Grand and Jackson County High School students, and community members wishing to further their education in the field of healthcare sciences.

Decide which scholarship best fits you and then click on the button below and fill out the online application. To request a printed version, please email foundation@middleparkhealth.org. Once your application is received, it will be reviewed by our scholarship committee. Applicants are generally notified of their approval status within one month of the date received. All applications must be fully complete and all applicants must be studying in the field of healthcare sciences to be considered. If a scholarship is awarded, we will pay your institution directly. We do not reimburse for past tuition.
High School Student
- Up to $2500 Scholarships for Grand or Jackson county graduating seniors.
- Must be accepted to any college/university or vocational school to further his/her studies in the field of healthcare sciences.
- Two letters of recommendation.

Scholarship Application Eligibility Requirements
- Before applying for an MPMF scholarship, applicants are required to utilize all Continuing Medical Education (CME) funds, if applicable, provided by MPH
- Physicians employed by MPH are not eligible to apply for MPMF scholarships.
- To be eligible, applicants must have completed a minimum of 1 year of service at MPH and should have the capacity to continue their work at MPH.
- Applicants must be currently employed at MPH and work a minimum of 30 hours per week.
- Applicants are required to provide two letters of recommendation, excluding any from family members. These letters should come from faculty, employers, or other community members who are familiar with the applicant's personal qualities and work habits.
- Applicants must submit their most recent performance evaluation received from MPH.
- Applicants should not be under a Performance Improvement Plan (PIP) at the time of application.
- All sections of the application must be completed in full. Please note that incomplete applications will not be considered.
Scholarship Awards and Service Commitment Guidelines:
- Employees who have served at MPH for a minimum of 1 to 3 years are eligible to apply for a scholarship, with a maximum limit of $3,000 per year. If awarded, the scholarship recipient must commit to serving MPH for a duration of 1 year. This service commitment period commences from the date when MPMF disburses payment to your educational institution.
- Employees who have dedicated a minimum of 3 or more years of service at MPH can apply for a scholarship, with a maximum limit of $6,000 per year. Scholarship recipients in this category are required to commit to serving MPH for a period of 2 years. The service commitment period begins from the date when MPMF makes the payment to your educational institution.
- If the employee fails to meet the service commitment years, as defined above, the employee will reimburse MPH for the prorated share of the funds paid to the educational institution, before his or her last day of employment at MPH.
