Middle Park Medical Foundation Employee Assistance Fund

 

Our healthcare is only as strong as the team that provides it, and everyone at Middle Park Health plays a role in the health of our community. For that reason, Middle Park Medical Foundation has set up an employee assistance fund for Middle Park Health employees who fall on financial hard times due to unforeseen and uncontrollable circumstances that require an urgent need for financial assistance. Examples include death in the immediate family, accidents, illness, natural disaster, a victim of a crime, etc.

Middle Park Health employees can apply for a grant of up to $2,400 from Middle Park Medical Foundation for financial assistance due to an unforeseen and unavoidable circumstance (triggering event).  Once the maximum amount is reached, the employee can no longer apply for assistance from that same triggering event. If a new and unrelated triggering event occurs, the employee is encouraged to apply for a new grant for assistance from the new triggering event. Documentation of event, expenses, and financial hardship is required. The applying employee must be a current employee with six months or more of service and in good standing with no performance issues within the past 12 months at Middle Park Health. Grants will be determined through an objective evaluation.

Select a dollar amount to be deducted from your paycheck each pay period.  Choose where you would like your gift to go.

Donate PTO by filling out the form and submitting to the MPH Human Resources Department.
